Coast Guard rescue swimmer dies after medical evacuation mission off Washington coast turns tragic

The U.S. Coast Guard on Friday confirmed Petty Officer 2nd Class Tyler Jaggers, an aviation survival technician stationed at USCG Air Station Astoria, Oregon, died Thursday following a medical evacuation mission off the coast of Cape Flattery, Washington.
During the Feb. 27 mission, Jaggers was critically injured and put on life support, according to statements from the Coast Guard and the Coast Guard Helicopter Rescue Swimmer Association (GCHRSA).
He later died at Madigan Army Medical Center in Fort Lewis, Washington, surrounded by his parents, sister, fiancé, friends, and Coast Guard teammates, according to Department of Homeland Security (DHS) Secretary Kristi Noem.
“We are grateful for his faithful service,” Noem wrote in a social media post.

Prior to his death, Jaggers was meritoriously advanced to AST2 for his “relentless drive in qualifications, the mentorship he consistently provided to swimmer candidates, and his exceptional performance across operational missions,” according to officials.
He was also awarded the Distinguished Flying Cross — one of the nation’s highest awards for heroism in aerial flight — by the Commandant of the Coast Guard.

“Through his selfless service and sacrifice, AST2 Jaggers exemplified the finest legacies of Coast Guard service, and the sacred oath of the Aviation Rescue Swimmer community: So Others May Live,’” officials wrote in the statement.
The GCHRSA said it is actively coordinating support for Jaggers’ family and the aircrew at Air Station Astoria.

Rep. Marie Gluesenkamp Perez, D-Wash., said Coast Guard crews “never waver in rushing to the water to save lives and that’s what Tyler Jaggers was doing when he was injured.”
“My thoughts and prayers are with him and his family, along with the entire United States Coast Guard and the team based in Astoria,” Gluesenkamp Perez wrote in a statement on X.

Driver in fatal Chinatown crash charged with vehicular manslaughter
Chinatown fatal crash victim ID’d, safety measures proposed
The victim killed in Friday’s Chinatown crash was identified Monday by the medical examiner as Cutberto Zamora-Martinez, 49, of San Joaquin County. At a meeting on Monday, city officials said the cause of the crash is still under investigation.
SAN FRANCISCO – The 76-year-old man arrested for a March 27 crash in San Francisco’s Chinatown that left a man dead has been charged with vehicular manslaughter.
Zhuo Ming Lu on Tuesday pleaded not guilty to the charges against him, and denied the allegations against him, according to the San Francisco District Attorney’s Office.
In addition to the charge of vehicular manslaughter, Lu is charged with driving a vehicle in the commission of unlawful acts and driving at unsafe speed without gross negligence.

Authorities said Lu was attempting to park near Grant Avenue and Jackson Street when his vehicle jumped the sidewalk and crashed into the landmark New Lung Ting Cafe, also known as the Pork Chop House. The vehicle struck two pedestrians: Cutberto Zamora-Martinez, 49, of San Joaquin County and a second person who has not been identified.
“The victims were transported by paramedics to a local hospital. Despite the lifesaving efforts of first responders and medical staff, one of the victims was declared deceased at the hospital,” a release from the district attorney’s office states. “Another adult victim was treated for non-life-threatening injuries.”

Zamora-Martinez had been working in the area, according to a GoFundMe page. A San Francisco Police Department source close to the investigation told KTVU the victims were carpet installers arriving for work.
The fundraising page described Zamora-Martinez as a husband and father who was the sole provider for his family and “a humble man who wanted the best for his family.”
Police said Lu remained at the scene of the crash and cooperated with investigators.

What’s next:
Lu was arrested in April, and was later released on his own recognizance. He was ordered not to drive, and to surrender his driver’s license and passport. The court also ordered the Department of Motor Vehicles to suspend Lu’s license.
He is scheduled to appear for a pre-trial hearing on Sept. 30.

Two Denver police vehicles hit by separate drivers during traffic stop
DENVER (KDVR) — Two Denver police vehicles were hit by separate drivers during a traffic stop last week, according to a social media post from the Denver Police Department.
On May 7, officers responded to a crash on northbound Interstate 25 near the exit for Alameda Avenue.
The officers were parked with their emergency lights on to direct the traffic away from the scene of the crash. Despite the lights being on, two separate vehicles crashed into the officers’ vehicles.
One driver caused minor damage to a Denver police vehicle and was cited for careless driving. The second driver caused extensive damage to a police vehicle and was cited for careless driving, as well as arrested for allegedly driving under the influence during the crash.
Officers were in their vehicles during the incident, with one uninjured and the other taken to the hospital with minor injuries.
“Traffic safety is a shared responsibility! If you come upon flashing lights, emergency vehicles, tow trucks, or disabled vehicles while driving, move over at least one lane or slow down if it’s not safe to move over. And don’t drive under the influence,” said the department in the post.
